Come and Join the Fun at Camp!
We are delighted that your camper will be joining us this summer! Please print out the information below so you and your camper will be prepared for a stay camp:
The forms MUST be brought with you to camp to be handed in during check-in. Campers will not be allowed to check-in without all forms on file.
Five Day (and longer) Resident Campers
Please print and fill out the following forms and bring them with you to camp.
- Parent Information Packet
- General Packing List
- Camp Tanasi Map
- Health History Form 1 (to be filled out by the parents/guardians)
- Health History Form 2 (to be completed by a licensed provider). All campers need to have had a physical within the past 24 months before coming to camp.
- All About Your Camper/It’s All About You
- Camper Pick-Up Authorization
- Vehicle Permission Slip (for for any camper taking an off-camp field trip)
- Asthma Form (if applicable)
- Girl Scout Registration Form (if applicable)
Three Day Resident Campers (Camper Sampler)
Please print and fill out the following forms and bring them with you to camp.

- General Packing List
- Camp Tanasi Map
- Health History Form for Short-Term Campers (to be filled out by the parents/guardians). No physical is needed.
- All About Your Camper/It’s All About You
- Camper Pick-Up Authorization

Your Way Campers
Please print and fill out the following forms and bring them with you to camp.

Game On! Campers
Please print and fill out the following forms and bring them with you to camp.

- Camp Wildwood Map
- Health History Form for Short-Term Campers (to be filled out by the parents/guardians). No physical is needed.
- Camper Pick-Up Authorization
- Asthma Form (if applicable)
- Girl Scout Registration Form (if applicable)
Enchanted Forest, Spa-la-la-la! and Splish Splash Campers
The registered adult will receive an email with more information about what to expect for the event and what to bring. This email will go out about three weeks prior to the event.
Payment
All forms and monies must be received either by mail (to the Knoxville Service Center) or in person prior to the camp session. Cookie Dough and Candy Cash will be applied to your camp balance when received. An invoice will be mailed to you with your remaining payment information if needed.
